This 1-hour workshop focuses on what to do before, during and after a healthcare appointment through action planning. Ultimately, patients are the most important members of their healthcare team.

Central West (CW) Self-Management Program is supported by the Ministry of Health and Long-Term Care – as part of the Ontario Diabetes Strategy. Its goal is to support people living with, or at risk for, chronic conditions by providing individuals with the knowledge, skills and confidence to effectively self-manage their chronic conditions and to improve their health. This is accomplished by providing self-management education and skills training to people living with, or at risk for chronic conditions as well as their health care providers.
